SUBMIT A DISCLOSURE
This form is to be filled out by inventors to inform the TCO of the nature on a new invention. This disclosure should list all sponsors of the research and should include all the information necessary to pursue protection and commercialization activities. It is critical that you complete every section of the disclosure in as much detail as possible or its processing can be delayed. You should also note the date of any upcoming publications or other public disclosure describing the invention. You may view a sample of either a life sciences or a physical sciences disclosure to help you fill one out in addition to following the appended instructions.When you are finished you may e-mail an electronic copy of the disclosure to our office but we will need a signed copy mailed to our office before we can begin to process the disclosure. This document will be treated as confidential.
